Coordinated Service Planning / Urgent Response
Coordinated Service Planning (CSP) is a collaborative and wholistic approach that brings together families, supports, service providers, Elder Knowledge Keepers, and community partners to develop a comprehensive plan that addresses the unique needs of the family. This plan includes all aspects of the medicine wheel with a focus on wholistic health, education, and social services.
The family plan is created with members of the family every step of the way and ensures services and support are accessible to achieve goals and improve overall family well-being. Coordinated Service Planning recognizes the importance of Indigenous culture, language, and traditions and promotes the involvement of Elders and traditional healers in the planning process. This approach is rooted in the belief that healing and support are most effective when they are culturally relevant and respect the unique heritage and beliefs of Indigenous families.
Coordinated Service Planner
A Coordinated Service Planner will meet with you and your family to better understand your current physical, emotional, spiritual, and mental needs. Together, you will create a roadmap connecting your family to critical support services in health, education, rehabilitation, and community.
Service Coordination ensures services are not only accessible but also integrated in a way that reflects family values and preferences, promoting self-empowerment and self-determination. By encouraging strong partnerships and open communication among all involved, Coordinated Service Planning aims to create a seamless network of care that enhances overall quality of life for Indigenous families.
