Access to your Personal Information/Disclosure Request

You have the right to your personal information which includes any written documentation about you and your family (with consent). You can make formal request in the following ways:

  1. Ask your worker if it’s a specific request (larger requests may be redirected to our Disclosure team)
  2. Use the online form below

Current Timelines

We are currently experiencing a high volume of requests which may cause delays. In order to speed up the process, please be very specific about the information you’re looking for. For example, some packages are upwards of 6,000 pages of information (depending on how long you and your family have been working with us.) So narrowing the request will help us and you get your information quicker!
